💉Nurse Note: Flu Campaign for November and December

👩🏼‍⚕️A note from Maresa C. Laxa, Wiesbaden Middle School Nurse

The Wiesbaden community Flu campaign has begun for November and December (see attached flyer for details on how to get an online appointment through the Wiesbaden clinic). Also see attached the Flu form (DHA 116) that you will need to bring with you on your scheduled day of appointment. The school's deadline compliance for the Flu vaccination is on Dec 01. 

Please take advantage of this event to have your student compliant by the deadline and avoid school exclusion. If you have any questions, you may contact me via e-mail at maresa.laxa@dodea.edu or by phone at 0611-143-545-2031.

If you are unable to make it to any of those Flu events, an appointment for Flu vaccination after the deadline is enough to satisfy the school requirement. If that will be your situation, please communicate your appointment date to me, as soon as possible.

If you choose to waive the flu requirements (due to medical or religious reasons), see attached waiver form. Complete, sign, and return to me before the compliance deadline.
